Confirmation of maternity leave and pay
Thank you for informing me of your pregnancy and your wish to receive maternity leave and pay. [As previously discussed,] you are entitled to 26 weeks ordinary maternity leave and a further 26 weeks additional maternity leave (52 weeks in total). You have indicated that you intend to commence your maternity leave on [date]. This means your maternity leave will end on [date].
If you want to amend the date on which you start your maternity leave you must, where possible, give me at least 28 days notice before either the new start date or your original start date i.e. [date leave starts], whichever is sooner.
