NB: Note for organisations using this template - New requirements came into force as at April 2020 regarding the statutory information which must be given to employees and workers on or before they start work.  There is limited guidance on the level of detail required for this at this stage and so, if using this template, you will need to keep its provisions under review in case of emerging case law or guidance which provides greater clarity on this point.  It is unclear if you are, for example, able to give details of hours and location prior to each assignment or whether you need to offer a new contract containing these details on each occasion.  Further, this template makes some assumptions regarding the worker’s engagement and may need to be tailored if these are not correct.  Please seek legal advice when using this template.
