Letter regarding death in service benefits

[        ]operates a Life Assurance Scheme under which the Trustees of the Scheme can exercise their discretion to make a payment upon the death of an employee.  [Your [INSERT] completed a Declaration of Wishes.  However, the Trustees would like to see a copy of your [INSERT] Will, if [s/he] made one, before deciding to whom to make a payment.] OR  [Your [INSERT] did not complete a Declaration of Wishes, therefore, the Trustees would like to see a copy of your [INSERT] Will, if [s/he] made one, so as to assist them in reaching a decision as to whom to make a payment.]  Please can you provide us with a copy, or let me know from whom I can obtain a copy.
